LaNelle Health Pro Management aligns the mission/ vision and activities of the home health agencies to that which comply the regulations of the Centers for Medicare and Medicaid Services (CMS), their Accrediting Organization (usually, The Joint Commmission) and the California Dept. of Human and Health Services, as governed by Title XXII of California.
The home health agencies that LaNelle Health Pro Managemement work with are offering intermittent visit services of highly skilled nursing, and PT/OT/ST /MSW professionals at the comfort of patients' own homes. It is important that patients consult their primary care physicians first and obtain a doctor's written referral along with documents that describe their homebound status to accurately determine the skilled home care discipline that the home health and/or hospice team needs to provide.

We train skilled nurses to become efficient home health and hospice care staff who possess valid California credentials and no criminal records at the time they join us.
HOW TO REFER TO LANELLE HEALTH PRO MANAGEMENT
Send us your contact number and information and a brief detail about what intermittent home health visit services do you need, if patient is private pay or Medicare covered, and the location of homebound patient/s. Our home health and hospice teams service Medicare eligible and private paying clients only. They will definitely require a doctor's written and signed order for all referrals. If you have no Primary Care Physician, LaNelle Health Pro Management can refer you to home visiting medical doctor groups free of charge for Medicare patients only. Private paying patient/s will be billed by the home visiting doctors' group for each home visit made, as in the case of our home health and hospice teams.
