Services Provided:
The Skilled Nursing Services are provided by Registered Nurses and Licensed Vocational Nurses with training and experience in providing care in the home. Our skilled nurses combine their hearts, hands and minds in providing holistic care for our patients.
Comprehensive Assessment and Evaluation, Teaching of Disease Processes, Post Surgical Care and Wound Care,
Medication Administration and Management, Intravenous Medication Management, Care of Ostomies, Insertion and Care of Foley Catheters, Medication Teaching.
Physical Therapy
is a service provided by Licensed Physical Therapists or Physical Therapists Assistants. They assess the physical capabilities and limitations of the patient, design personalized home exercise programs to meet individual patient’s rehabilitative goals.
Development of Therapeutic Exercise Program, Balance/Coordination, Gait/Transfer Training, Increase in Mobility and Ambulation, Pain Management, Prevention of disability following diseases or loss of extremities, Teaching and training on the safe use of assistive devices, Weight-Bearing Activity Training, Safety Education.
Occupational Therapy
is a service provided by Licensed Occupational Therapists or Certified Occupational Therapist Assistants. They are able to assess the patient’s ability to perform self care activities. Therapy program may then be designed to achieve maximum independence in day to day activities.
Development of home exercise program, Restore / improve use of upper extremity, Assist patient to achieve maximum independence in daily living activities (ADL/IADL), Teach and train safe use of corrective / support devices, Safety Measures.
Speech Therapy
is provided by Licensed Speech Therapists.The ability to communicate is essential to an individual’s self worth. Our Therapists will work to restore our patient’s self confidence by aiding in the abatement of speech disorders. They will analyze speech and associated problems that may result from illness, aging or disability and will assist patients with impaired communication skills to regain or learn alternative ways to express their needs and feelings.
Traumatic brain injury, Stroke, Cranial nerve damage, Feeding and swallowing difficulties, Safety Measures.
Home Health Aide Services are provided by Certified HHAs.
Bathing the patient, Shampoo, skin and mouth care, Range of motion exercises, Linen change.
