Skilled Nursing

Skilled nursing and clinical services in the home setting to care for patients with special needs, such as: Cardiac Care, Diabetic teaching & monitoring, IV therapy, Case Management, Per & Post-Operative Care, wound care, geriatric care. Synergy screen our nursing staff for proper credentials and compassion to service our patients.

Physical Therapy

Our Physiotherapy personnel provide assessment, treatment and education of movement problems that result from pain, disease, accident, injury, aging and lifelong disabilities. This may include pain management alternatives for acute and chronic movement disorders, as well as functional mobility assessment and comprehensive neuron- rehabilitation services for traumatic episodes.

Occupational Therapy

Occupational therapist strides to ensure that the patients return to his/her maximum level of functionality so that he/she can safely participate in daily activities of self care, work and leisure. Occupational therapist also educate the patient/family in adaptive techniques which will help the patient become self sufficient.

Speech Therapy

Speech language Pathology provides assessment, counseling, and treatment in speech, language disorders, swallowing difficulties, and augmentative and alternative communication and treats those disorders. The speech therapist can also work with the family to cater to a patient’s direct need of treatment care.

Medical Social Worker

Synergy select professionally trained Social worker’s to assists individuals and the families to cope with the stresses (psychological, emotional, and or financial) that often accompany illness. Social Workers counsel families in crisis and help locate financial and mental assistance for those in need.

Home Health Aid

Our certified home health aids are trained to perform home health care services needed by our patients. The home health aide will be assigned in a manner which promotes quality, continuity and safety of patients. The companionate attitudes of our home health aides make a real difference in patient’s life and comfort.

Dietitians

Synergy also have dietitians on staff for our patient’s nutritional needs. Dietitians are responsible for crafting the diets of patients and for educating individuals on the benefits of maintaining proper daily dietary standards. Dietitians work in a variety of different positions depending on their particular specialization. Dietitians often work closely with other professionals in order to design menus and diets that are the most beneficial to their patients.

Admission Consultant

Synergy admissions coordinator is responsible for the development of admission statistics. He/She serves as a link between the organization and patients/families. The admission consultant handles any inquiries, complaints and concerns from patients. He/She works with various members of staff with the purpose of ensuring that all requirements are fulfilled before admission takes place. He/She ensures that admission processes are completed within the designated time and all documentation is presented. An admissions coordinator can also be called upon to organize special events that serve the purpose of promoting the organization.
