Services provided are Skilled nursing care, Physical Therapy, Speech Therapy, Occupational Therapy, Nutritional services - Dietitian, Medical Social Worker and Certified Home Health Aids. Admission to Elsmar can only be made under the direction of a physician, based upon the patient's identified care needs, homebound status and the type of services required that we can provide. The cost of specific home health care services are covered by private/group insurance, Medicare, Medicaid. Our Mission at Elsmar is to provide individualized home care services to maximize the health, independence, dignity and comfort of our patients and their families. These services are provided by an interdisciplinary team that assists with managing the patient's care throughout their home care recovery process.
